- $399 for 12 consecutive months of coverage
- 2 exams for 12 consecutive months
- 1 limited exam for 12 consecutive months
- 2 cleanings for 12 consecutive months
- Any necessary x-rays for 12 consecutive months
- 2 fluoride varnish treatments
- 15% discount on all dental treatments for 12 consecutive months
Plan B
- $599 for 12 consecutive months of coverage
- Tailored for periodontal disease maintenance
- 2 exams for 12 consecutive months
- 1 limited exam for 12 consecutive months
- 4 periodontal maintenance procedures for 12 consecutive months
- Any necessary x-rays for 12 consecutive months
- 2 fluoride varnish treatments
- 15% discount on all dental treatments for 12 consecutive months
Plan C
- $325 for 12 consecutive months of coverage
- 2 exams for 12 consecutive months
- 1 limited exam for 12 consecutive months
- 2 cleanings for 12 consecutive months
- Any necessary x-rays for 12 consecutive months

- 15% discount on all dental treatments for 12 consecutive months
* All plans must be paid in full the day the contract is signed. Membership plan is valid for 12 consecutive months from the date the contract is signed. No exceptions. May not be combined with any type of insurance plan or fee schedule. Based on the current care you receive with us now determines the plan you fall under when signing up
