New Patients
Your first visit is designed to get you better acquainted with Dr. Fleming and our caring staff. We encourage questions and will always do our best to deliver top-notch quality care.
Once the initial registration and medical history forms have been completed, you will receive a comprehensive exam, complete with x-rays, head and neck cancer screening, a periodontal evaluation and baseline dental charting.
Dr. Fleming will then discuss the current state of your dental health and develop a plan for your dental needs in the future.

First Appointment
Yes. Please arrive 10-15 minutes early to fill out any remaining patient forms.
It varies, but please plan on 1 to 1.5 hours for the first visit.
Please bring the following items with you to your appointment:
- Dental Insurance Card (if applicable)
- Identification such as Driver’s License, Military ID or State ID
- New Patient Registration Form
- Patient Medical History Form (if applicable)
- Financial Policy Form
- HIPAA Consent Form
